New Patients
We will have you fill out some intake paperwork at your 1st first visit. We ask that you arrive 15 mins early to allow time to complete paperwork.
Once the patient is checked in, healthcare professional will conduct an evaluation and provide consultation.
Our office does not participate in insurance company assignments. Upon request, we provide a receipt for your records, which you may submit to your insurance company for reimbursement. However, we do not directly handle paperwork submissions to insurance companies.
We accept cash, checks, HSA cards and major credit cards.
